The Diahnosis of Leprosy by the Sternal Puncture


Abstract
The author examined 200 patients with leprosy (Hansen¢¥s Disease) using one of the new methods of saponin concentration with the following results:
1. The bone marrow obtained by sternal puncture was allowed to stand 24 hrs at room temperature. The leucocyte layer on top and the suspension layer were examined. More leprosy bacilli were found in the suspension layer than from the leucocyte layer.
2. When an antiformin concentration method was used the number of leprosy bacilli found was not increase.
3. Using the saponin concentration method leprosy bacilli were found 91% of patients of the tuberculoid type, 71% of the lepromatous type and in 3 out of 10 cases of maculo-anaesthetic leprosy.
4. Sodium iodide was¢¥ administered to 20 patients in whose bone marrow no leprosy bacilli had been found. When the symptoms recurred the bone marrow was again examined with the finding of only one positive case out of the 20 patients.
.0. The percentage leprosy patients in whom bacilli were found in bone marrow was highest in cases of the lepromatous type, next highest in cases ¢¥of the tuberculoid type and lowest in the maculoanaesthetic type.
There was no corelation between the percentage of discovery rate and the clinical severity of the disease.
6. There was no corelation between the percentage positive cases discovered and the duration of illness. There seemed to be a higher percentage in the early years (1-4 years) in the lepromatous type.
7. The rate of discovery of bacilli was higher in little treated than in patients who had received more treatment.
2. The rate of discovery of leprosy bacilli was high bone marrow ¢¥examination in all types of leprosy and even higher when treated. with saponin and concentrated.
The saponin method of detection of leprosy bacilli seems to be of value in, the diagnosis of leprosy especially in the early stage.
It is also of value in providing information regarding the preset de of -leprosy bacilli in the body.
